PAYMENT POLICY

Payment is due in full at time of service unless other arrangements have previously been made. Our office accepts cash, check (once an established patient), MasterCard, Visa, American Express, debit, HSA (Health Savings Accounts) and FSA (Flex Savings Accounts). All patients are required to provide a valid credit card on file to schedule and confirm appointments. The card on file will not be readily available for payment at the time you receive treatment. It is used to confirm and guarantee payment for appointments only. HSA and FSA cards will not be considered for this purpose.

APPOINTMENT POLICY

We require 24 hour notice for any cancellation or appointment changes. Less than 24 hour notice will result in full fee charged to the patient. Emergency situations will be given special consideration. Please call Life Choice Chiropractic to cancel appointments. Text and email cancellations will not be received by the scheduling department and will result in a cancellation charge.

Please arrive 5 to 10 minutes early to fill out the patient intake form on our iPad. If you are a new patient, please allow for 20 to 30 minutes of filling out patient information, insurance information, etc. Our office uses an electronic platform for all records. This ensures HIPAA compliance and EHR compliance per Medicare standards.

Late arrivals will not receive an extension on their scheduled appointment time. Please plan ahead for traffic and parking. We reserve the right to reschedule your appointment time if you are more than 5 minutes late as to guarantee the chiropractors do not run behind with their other patients. We believe that your time is valuable and do not want to waste it making you wait longer than necessary for your treatment.

INSURANCE POLICY

We do accept most insurance and may be in-network with your carrier. We cannot guarantee that services are covered as every individual's health insurance plan varies. Insurance verification will happen once you're in the office and we will do our best to inquire about any services the chiropractor may deem necessary for care. Some services may not be covered by certain health insurance plans.
